Your initial appointment is required to be in office. After this appointment, you may discuss with your provider about scheduling telehealth appointments.
Cancelation Policy
Please be aware of our cancellation policy- you must call and cancel your appointment at least 24 hours prior to appointment time in order to avoid cancellation fee. To Cancel your appointment at anytime please call 234-368-2772.
